NEW PATIENTS

Unfortunately, we have decreased our rate of acceptance of new patients! With limited staff our first concern is serving our established patients. For the reason while we are accepting new patients we take them on only a few at a time, and have slowed the pace of the process as well. If you are needing to transfer care urgently, our office may not be the best choice.

We are first accepting newborn infants who (1) have not already been established elsewhere AND (2) have been pre-registered* with our office prior to birth.

(*Pre-registration is REQUIRED, or you will not be permitted to join. Our busy schedule demands that we reserve space for newborns - this is ONLY possible if pre-registration is completed. If your baby was just born, and pre-registration has not already been completed, unfortunately we cannot accept your child.)

We are working from a wait list for any family interested in joining us down the road.

To be added to the wait list please send us a “wait list request” email including (1) a copy of any current insurance coverage, as well as (2) ages and (3) a brief description of your child(ren)’s medical needs.

Under normal circumstances: all children are welcome at our practice, including new born babies, toddlers, school age children, and children with special needs. If you would like to join our practice please review the steps below.

INSURANCE

Please first send us your insurance card(s) for approval. We require copies or photos of all primary, secondary, tertiary, share plan, and/or Medicaid coverage cards (front and back). Please do not exclude any insurance information, as we cannot advise you if we are not fully informed. We cannot register (or schedule any visits) until we have completed this insurance/billing screening process, and registration forms are on file. Please review our Insurance page for more information on our insurance policies.

If (but NOT before) we approve your insurance, you may complete the corresponding patient forms provided below and send them to our office!

TRANSFER MEDICAL RECORDS TO OUR OFFICE

To release records FROM your current office, specialist, or hospital TO Carine Pediatrics, please complete the Medical Release of Information Authorization Form.  Please be sure to detail any necessary specifics and complete ALL FIELDS on the form. Call with the form in front of you if you need assistance.
